AGREEMENT ON RESPONSIBILITIES AND FINANCIAL MATTERS FOR EDUCATIONAL ACTIVITY SITE COORDINATORS
The following agreement is set forth by and between The American Heart Association (AHA), (Sponsor) and Spotlight Series Event Host (Site Coordinator) for the CME/CE activity entitled Spotlight Series. The date and location for this CME/CE activity is to be determined. This Agreement concerns only the roles and responsibilities of the Sponsor and Site Coordinator and their respective financial fees and costs.
Final Responsibility and Authority
As the Sponsor, the AHA is the final arbitrator on all matters regarding this CME/CE activity and it is responsible to the ACCME for the management and operation of this activity in accordance with all rules and regulation of the ACCME and their Essential Areas and policies.
Accreditation
The AHA is the sole provider of CME/CE credits for this event.
Speaker
Speakers are assigned based on availability and location. Once a speaker is selected and approved, the Site Coordinator will be notified. AHA-approved speakers are certified to speak on all content related to their approved series.
Outcomes Surveys
In an effort to evaluate the effectiveness of the Spotlight Series program, all program participants are subject to being contacted to participate in a post-event assessment survey conducted by Outcomes, Inc. As such, participant contact information including: name, specialty, and e-mail are essential to the program. One Attendance Verification Form should be given to each participant, along with the attendee handout, prior to event start and must be used to capture program participant data. All data captured on Attendance Verification Forms must be entered on the Spotlight Series website within 5 days of event completion. CME/CE credit is contingent upon the receipt of participant data.
Role and Responsibility of Site Coordinator
We greatly appreciate your help and involvement in this important educational series. To help ensure a successful educational experience for those in attendance, we require that Site Coordinators perform the following functions with the approval and direction of the Sponsor:
- Contact speaker prior to event to review institution location information, on-site direction and technical needs.
- Utilize Attendance Verification Forms to collect attendee data and enter information on Spotlight Series event page within 5 days of the completion of the event. Participant CME/CE is contingent upon receipt of attendee contact information.
- Provide Attendee Handouts (available on event page at www.spotlight.heart.org) to all attendees. Assist attendees with copies of slide presentations, as needed.
- Manage the on-site activity.
- ASSURE THAT NO COMMERCIAL SUPPORTERS ARE ASSOCIATED WITH THE PLANNED EVENT (I.E. PROVIDING REFRESHMENTS).
- Consent to be primary contact for attendee inquiries surrounding your program’s on-site logistics.
Responsibilities of the AHA (as Sponsor)
The Sponsor’s duties and responsibilities include, but are not limited to, the following:
- Assure compliance with all applicable ACCME Essential Areas and policies, and other policies and regulations of the AMA and FDA with regard to accredited continuing medical education activities
- Oversee and manage the Site Coordinator for compliance with the above
- Management of online evaluation process and issuance of CME/CE credit certificates
- Issuance of faculty honoraria and arrange and financially support speaker travel
Financial Matters
It is agreed all faculty honoraria and related travel expenses will be paid for by the American Heart Association. The Site Coordinator is responsible for any fees associated with on-site expenses such as printing and refreshments.
